Providence RN Navigator Orthopedics/Alternative Payment Model in Walla Walla, Washington
Providence is calling a Registered Nurse (RN) Navigator Orthopedics/Alternative Payment Model (Hospital Administration, Days, 0.5 FTE) to Providence St. Mary Medical Center in Walla Walla, Washington. In this position you will:
Plan, coordinate and provide joint expertise and consultation by overseeing the complete care of the total joint replacement (TJR) patient.
Serve as a clinical resource and 'point person' for care coordination, and continuity, overseeing pre-op surgery preparation, education, an individualized care plan, discharge plan, and post-acute follow-up:
Work collaboratively with health plans, finance, and facilities to develop integrated, coordinated systems of care for patients to experience connected, seamless care.
Maintain accurate census and monitoring of TJR patients, including those in APM.
Ensure appropriate pre-op preparation for each patient prior to surgery; conducts a comprehensive assessment to inform appropriate risk stratification, care pathway selection and resource utilization.
Identify patient needs, amenities and coordinates necessary resources to administer the services
Participate in rounds to ensure compliance with practice standards, address discharge barriers, monitor ancillary services utilization, and conduct patient interviews
Review patient charts for document compliance to standard TJR protocol and individualized care plans and education; identifies gaps and barriers in treatment plans to ensure patients receive appropriate resources and services.
Coordinate effective transitions of care and effective handoffs to the next level of care; performs outreach to SNFs, Rehab and Home Health to ensure patient needs and key metrics are being met
Follow up with patients at pre-determined intervals and as needed; communicates status with the patient’s care team as appropriate
Maintain awareness of key performance metrics and appropriate management of costs; coordinates interventions to prevent adverse events such as ED visits and hospital readmissions.
Serve as a professional resource for hospital departments, physician offices, post-acute partners outreach networks and APM payer contacts to coordinate care services:
Develop/review standardized care plans with the surgeons including: pre-op, intra-op, post-op and discharge order sets
Maintain close, collaborative and consulting relationship with all disciplines who provide and/or support patients receiving TJR care (e.g., nursing, case management, therapies, pharmacy, emergency room providers, patient access, inpatient rehab, volunteers, primary care and post-acute staff, etc.)
Serve as patient/family advocate with physicians, health care staff and/or administration
Serve as an advocate for continuous process improvement and leads interdisciplinary teams toward accelerated process improvement:
Oversee and incorporate patient reported outcome data collection and program-approved digital interface tools.
Actively participate in the TJR CoE certification process.
Provide TJR Process Improvement Team leadership for ongoing quality improvement.
Identify areas for improvement/conduct continuous process improvement and patient satisfaction initiatives.
Ensures a measurement system for tracking surgical outcomes, patient satisfaction, and financial metrics.
Prepare TJR cases for review.
Provide key statistics to the O&SM Regional Service Line Director. Statistics may include: clinical outcomes, patient satisfaction/survey results, cost data, canceled surgeries, LOS, readmissions, denied days, etc
Coordinate Joint Care Patient Advisory Councils with patient experience.
Collaborate with Clinical Education to develop, coordinate and provide patient, staff and community education:
Develop and/or revise service-specific policies, procedures and protocols and ensures staff are educated accordingly
Oversee/ coordinate pre-surgery educational offerings and content for patients and their care partners
Oversee the development and on-going editing of service-specific forms and patient education materials such as: patient guidebooks, in-room communication materials, discharge checklists and surveys, and other standardized materials specific to the service line
Develop/participate in public, professional education programs to further knowledge, expertise, and awareness regarding conditions that may lead to Orthopedic specialty interventions and/or current best practice protocols for total joint replacement care.
Enhance professional growth and development through literature reviews, seminars, conferences, etc. as related to TJR. Shares learning with others.
Required qualifications for this position include:
Graduate of an accredited RN Program
Current WA State RN Licensure.
Current valid driver’s license.
Two (2) years recent clinical experience in an acute or ambulatory care orthopedics setting that includes orthopedic care
Professional Orthopedic specialty certification (or equivalent) required within 2 years of hire date.
Preferred qualifications for this position include:
Case Management experience or Population Health background
Proven working relationship with physicians, clinic personnel, and medical center staff, with demonstrated success in relationship building.
Proven leadership in continuous performance improvement and change management; including metric collection and reporting.
Experience in education content program development and presentation.
Current knowledge of alternative payment models.
Bi-lingual or multi-lingual skills.
