Providence Health & Services Community Health Worker in Spokane, Washington


Providence is calling a Community Health Worker (Population Health, Day shift,1.0 FTE) to Physician Management Group in Spokane, Washington. In this position you will:

  • Provides quality service in the performance of work assignments and duties

  • General Outreach Duties:

  • Actively seek opportunities for outreach with a focus on hard to reach populations and our identified “high-need” communities.

  • Conduct presentations about available services to local community based organizations

  • Maintain and strengthen relationships with key community stakeholders.

  • Work with other caregivers within the Population Health Department to coordinate outreach events and direct service opportunities; communicate outreach events

  • While performing outreach activities, recognizes possible community resources and brings back information to supervisor

  • Maintains knowledge of Providence Population Health programs and provides community with appropriate linkages as requested

  • Acts as representative for Providence Health and Services at professional and network meetings, as needed.

  • Attend and participate in regular staff and department-wide meetings.

  • Health Education Duties (for CHWs in the Health Education Track):

  • Identify community members/patients who would benefit from the health education courses

  • Refer identified community members/patients to caregivers that run the respective program

  • Assist caregivers with administrative tasks associated with the implementation of the health education courses

  • Identify materials appropriate for the target audience and submit materials to Supervisor for approval before being distributed in the community

  • Enrollment Duties (for CHWs in outreach and enrollment track):

  • Provide outreach in the community to increase awareness of Medicaid and SNAP (food stamps)

  • Administer screening tool to identify and refer community members/patients who are eligible for Medicaid and SNAP (food stamps)

  • Navigation/Linkage to Resources Duties (for CHWs in the Navigation/Linkage track):

  • Provides on-site screenings of risk-factors that impact an individual’s well-being in community settings.

  • Refers clients/patients to the appropriate programs in the community that meet the needs identified in the screenings

  • Builds relationships with clients/patients by asking appropriate questions in a compassionate manner and uses good listening skills

  • Follows up and assists families who are having difficulty accessing resources to which they were referred

  • Administers client satisfaction survey to clients assisted

  • Fully understands and is able to explain the issues for which they are screening and the resources available to meet those needs

  • Works closely with Population Health and Medical Center caregivers to ensure a seamless flow for the patients

  • Documentation/Information Management:

  • Document all outreach events and direct services including maintenance of logs and sign-in sheets

  • Implement and monitor collection of program’s evaluation data (e.g. surveys)

  • Ensure that program statistics are entered into the appropriate database (e.g. Carescope or other Population Health database) per department deadlines.

  • Submit monthly reports to Supervisor and/or Department Data Specialist

  • Other duties as assigned

  • Development:

  • Develop, maintain and demonstrate up-to-date knowledge on Population Health issues by attending seminars, trainings, or conferences as required by the Department, reading materials or other methods

  • Develop, maintain, and demonstrates up-to-date knowledge of hospital and community resources

  • Participate in staff development activities

  • Human Resources:

  • Employee completes all tasks and duties accurately and timely within assigned shifts. Demonstrates initiative to perform job duties autonomously

  • Attends Mandatory and other education offerings per department and organizational requirements. May assist with education within the department


Required qualifications for this position includes:

  • Valid Driver’s license, reliable vehicle and auto Insurance

  • Related Community Experience

Preferred qualifications for this position includes:

  • Associate's Degree in Health or Social work related field.

  • Bachelor's Degree in Health or Social Work Related Field

  • Preference will be given to applicants with lived experience

  • Bilingual English /Spanish
