The Home-Based Acute Care Occupational Therapist is responsible for therapy evaluations, treatment planning, implementation, and discharge planning for inpatient hospital at home patients in the home setting in accordance with established policies while maintaining the highest degree of quality patient care. As a member of the multi-disciplinary team, the therapist assumes responsibility for communication of patient-related issues with appropriate team members or other staff, and participates in collection and evaluation of data necessary to assess appropriateness of care at home. The Occupational Therapist may also be assigned to orient staff and students. The Occupational Therapist works with the patient and family/caregivers in the home setting without direct supervision.

In this position you will:

  • Demonstrates competency in providing quality therapy services including assessment, intervention, program planning and implementation, patient/family/caregiver education, and discharge planning for a wide range of diagnoses. Demonstrates knowledge of contraindications for therapy evaluation/treatment. Develops a therapy plan of care in collaboration with patient/families/caregivers. Assesses effectiveness of treatment and modifies treatment/plan to achieve goals.

  • Evaluates physical and functional status as well as cognitive, psychological and perceptual function to determine the patient’s capabilities to function safely at home. The home and family/caregiver are assessed for safety risks, and ability to support and care for the patient at home.

  • Provides care in accordance with physician orders and the established plan of care.

  • Documents all evaluations, treatment and coordination of care contacts in appropriate format. Meets time guidelines for completing documentation. Documentation reflects clinical skill as well as meeting billing and reimbursement requirements.

  • Demonstrates skills, knowledge, abilities, training and education to serve the following age groups: adult and geriatric.

  • Treatments are adapted to the unique home setting and to the patient and/or caregiver's ability to follow through safely and correctly. Requests consultation, supervision, or continuing education to learn specialized techniques or treatment approaches.

  • Communicates pertinent medical and clinical information (medical, social, and emotional factors) to the Interdisciplinary team, including information regarding the patient’s care plan and implementation of changes per physician order. Also communicates pertinent information to team members, supervisor, staff and external sources as appropriate.

  • Plans for discharge. Participates in team reassessments and discharge planning. Teaches patient/family/caregiver discharge therapy program. Recommends follow up rehabilitation therapy needs

  • Evaluates, recommends and assists in obtaining adaptive equipment. Adapts equipment and techniques for use in the home setting, ensuring optimum function and safety. Provides consultation or makes appropriate referrals regarding resources in the community.

  • Independently manages own case load, plans and schedules home visits in coordination with other team members as needed to maximize effectiveness of overall care plan. Organizes schedule to attend required meetings. Responds to urgent patient needs by prioritizing and triaging treatments. Communicates schedule and work plan to scheduling coordinator, supervisor and other team members as appropriate.

  • Educates/consults with hospital at home staff, hospital staff and community resources regarding therapy services in the hospital at home setting. Demonstrates a working knowledge of all available hospital at home services in order to make appropriate referrals for those services. Provides in-services to staff regarding new treatment/techniques learned.

  • Participates in in-service programs and community continuing education needed to remain clinically current in evaluation and treatment techniques.

  • Provides supervision for Certified Occupational Therapy Assistants and students per regulatory guidelines.

  • Participates in the orientation of new staff members by orienting them to the unique clinical practice of their discipline in the hospital at home setting.

  • When providing services for patients in settings other than a private home (e.g., Adult Family Home or Nursing Home) coordinates care, provides consultation and training, and meets additional documentation requirements specific to that setting.

  • Attends required team meetings. Participates in agency committees as assigned by supervisor. Participates in facility/agency program planning, gives input to program policy discussions particularly in those areas impacting client care.

  • Supervises students assigned by the supervisor.

  • Follows facility/agency communication Policies and Procedures for voicemail, reporting schedules and schedule changes, responding to pagers and notification of information to supervisor and others.

  • Maintains confidentiality of all client demographics, medical and financial information at all times. Maintains confidentiality of all company and client information. Ensures proper handling and disposal of confidential documents and adheres to all HIPAA rules and regulations.

  • Adheres to all policies and procedures including regulatory requirements with regard to infection control, practicing universal precautions, sanitation, and safety and disaster.

Qualifications:

Required qualifications for this position include:

  • Education sufficient for licensure

  • 1 year of therapy experience with adult geriatric and/or individuals with physical disabilities

  • Current Washington State Occupational Therapist License

  • Valid Driver’s license

  • Current CPR certification required within 30 days of hire.

  • Requires access to daily use of an insured reliable vehicle or an alternate ability to travel to sites as required.

Preferred qualifications for the position include:

  • 1 year of therapy experience in home care or in related rehabilitation setting

  • Experience with the geriatric population
