RN Case Manager Providence Elderplace

The Registered Nurse Case Manager establish a well rounded care practice that includes: a patient assessment, care planning, implementation, coordination, and monitoring and ongoing routine evaluation that ensures medically and financially appropriate care. Responsibilities include: facilitating interdisciplinary team care coordination, intensive medical management of participants recently discharged from hospital and skilled nursing facilities. Additionally, the RN CM will work closely with the IDT Social Worker and Physician, to collect the necessary medical and social information to ensure comprehensive decisions are made which ensure the safety and well being of participants.

Required qualifications:

  • Graduate of accredited school of nursing

  • Current Washington State Registered Nurse (RN) license

  • Current Cardiopulmonary Resuscitation (CPR) certification

  • Current Washington State drivers license

  • Proof of auto insurance

  • One (1) year of long term care or geriatric nursing experience

Preferred qualifications:

  • Bachelor's Degree in Nursing

  • Current CCM, NACCM, ACME

  • Medical Surgical Experience or Rehabilitation Medicine
