Providence Health & Services Senior Program Manager - Care Management /Community Health Worker Programs in Renton, Washington

Description:

Providence St. Joseph Health is calling a Senior Program Manager Care Management/Community Health Worker Programs to any location within our Providence footprint or any approved remote location.

We are seeking a Senior Program Manager Care Management who will function as a resource for supporting process and skills improvement for care managers in all care settings to facilitate data-driven, whole-person oriented, proactive care management for patients identified as most likely to benefit from care management interventions.

The position will lead and facilitate multi-disciplinary, cross-functional, and cross-regional teams, initiatives and projects of varying size and complexity and is viewed as a partner to regions in implementation, optimization, and standardization of data, evidence, and operations informed practices. The position will facilitate virtual teams from concept phase through implementation, flexibly responding to needs and enhancing the internal capability within the organization to develop and drive change, facilitate alignment, and demonstrate return on care management activity. The position requires excellent organizational and communication skills, and an ability to organize activities and move initiatives forward in an ambiguous environment and actively participates in all ECMC meetings regarding care management practices and workflow development, as well as ensuring consistent documentation for care coordination occurs.

This position is a senior level position with advanced knowledge. The person in this role will serve as a mentor and coach for skills development for team members as needed. The person is this role will provide project, technical, and process guidance to team members and project participants as needed.

In this position you will:

  • Development of community health worker pathways/interventions, collaborating with ECMC, care management, Community Health Investment and Pop Health leaders across PSJH

  • Develop workflows, process documentation, flow charts and procedure documentation for community health worker programs in collaboration with ECMC, care management leaders, caregivers

  • Responsible for the planning, facilitation, and deliverables for workgroups specific to, and in support of, community health worker program standardization

  • Serve as subject matter expert for community health worker program optimization

  • Identify and resolve problems or difficulties while retaining strong and effective working relationships staying calm and objective under the stress of deadlines and obstacles

  • Design, develop, and deliver presentations to community health worker leaders and staff; must be comfortable with public speaking and presentations

  • Provide project, technical, and process guidance to team members and project participants as needed.

  • Manage several projects, work streams, and/or deliverables simultaneously, balancing needs of community health worker leaders or other key stakeholders to ensure their programs are supported by enterprise efforts

  • Serve as a mentor and coach on best practices to regional partners and care teams, aiming to help improve the team's overall expertise and performance.

  • Develop and maintains strong relationships with all system and regional leaders and care teams, as well as other shared services leaders. Leverages these relationships and interactions to elevate internal best practices and drive standardization across the enterprise

  • Partner with regional and system data analytics and informatics teams to monitor and evaluate program outcomes

  • Support strategic planning and partnership building for ongoing community health worker program development

  • Perform other duties and special projects as required and assigned.

Qualifications:

Required qualifications for this position include:

  • Bachelor's Degree in related discipline or equivalent educ/experience

  • Coursework/Training: Process Improvement, Change Management, Project Management, Operational Excellence, Lean or other similar methodology or equivalent experience

  • 6 years experience in Administration, Clinical Leadership, Quality, or Training

  • 6 years experience with applying process improvement and change management methods, and/or developing programs

  • 4 years experience in a clinical role or in a healthcare setting and / or documenting within an EMR system (i.e. Epic)

  • 6 years relevant experience with Microsoft Office applications (Excel, Word, PowerPoint)

Preferred qualifications for this position include:

  • Master's Degree in related discipline or equivalent educ/experience

  • Experience with Community Health Worker Programs

  • Training related to health equity, social justice, cultural humility, etc.
