Providence St Joseph Health is calling a Population Health Clinical Coordinator to our location in Renton, WA. This position will be virtual with a need for onsite meetings as needed. Apply for immediate consideration via virtual interview process.

We are seeking a Population Health Clinical Coordinator to be responsible for coordinating multiple aspects of care for patients with preventative care needs, chronic diseases, and care opportunities within the Physician Enterprise. Daily work includes ordering laboratory tests or imaging exams per protocol to facilitate patient screenings, management of preventative care, and supporting optimization of chronic disease treatments as prioritized thru Enterprise Clinical governance. The Population Health Clinical Coordinator assures that an integrated approach to the patients’ care coordination is fulfilled. This position is the liaison between Physician Services Enterprise providers, primary care clinics, specialist offices, patients, multiple Providence St Joseph Health scheduling and ancillary departments (e.g., Laboratory), and freestanding care delivery centers or services for population health programs and related processes.

Performs all duties in a manner that promotes team concept and reflects the Sisters of Providence mission and philosophy.

In this position you will:

  • Triage clinical disease registries in Epic and prioritizes coordination of patient’s chronic and preventative care needs.

  • Act as a resource and contact for the Physician Enterprise Population Health Program and provides technical assistance to providers, clinical support staff, and/or Clinical Pharmacy Specialists. Serves as a liaison and resource to the clinic staff and the patient for optimizing the coordination of care and immediate problem resolution. Collaborates with other team members to ensure the best patient outcomes.

  • Order laboratory tests or imaging exams per protocol to close care gaps.

  • Request and abstract clinical data (eg. immunizations, laboratory tests, imaging studies, procedures) in Epic from external and/or other EHR sources.

  • Coordinate the Disease Management with Primary Care Provider scheduling processes for medical group patients in a centralized department setting.

  • Use scheduling systems and understands different protocols for laboratory services at multiple Providence St Joseph Health facilities and freestanding laboratory centers within specified timeframes.

  • Navigate data system(s) to research and/or record pertinent information in compliance with policies. Reviews patient chart to obtain documentation of requested services.

  • Receive and appropriately addresses phone calls from patients. Identifies urgent or emergent situations and responds appropriately.

  • Utilize motivational and coaching techniques as necessary to remove barriers to care and engage patients in their care.

  • Use patient engagement tools such as, phone calls, letters, MyChart messages, and/or IVR solutions.

  • Work with the healthcare team to identify and continuously improve the quality of services provided and actively collaborate in finding solutions.

  • Process all paperwork/forms related to laboratory scheduling.

  • Contact specialist offices to obtain clinical information (chart notes, diagnostic results) as needed to assist in disease management process.

  • Facilitate updating of patient registration information if inaccurate or no longer valid.

  • Assist providers with appropriate ICD10 diagnosis coding as needed to comply with insurance coverage guidelines of various labs.

  • Perform other work-related duties and special projects as assigned.

Qualifications:

Required qualifications for this position include:

  • Completion of an accredited Medical Assistant program and registered with State Board (where required by state) and/or certified through a nationally recognized Medical Assistant program such as AAMA, AMT, RMA, CCMA or NCCT or Nationally Certified Pharmacy Technician or registered with the State Board

  • High school diploma or GED

  • Requires knowledge of medical terminology, disease processes, medical decision making, and prior experience in utilizing disease registries or similar databases

  • Requires excellent verbal communication and interpersonal skills, with preferred experience in motivational interviewing / coaching

  • Experience with electronic medical records and/or computerized billing systems

  • Epic or other Electronic Health Record /care management software experience

  • Computer skills including Microsoft Windows and Office Applications (Word, Access, and Excel)

  • Excellent typing skills (60 words/minute), computer keyboard, and mouse skills for expedited data processing

Preferred qualifications for this position include:

  • College Degree

  • For Medical Assistant, a minimum of 1 years of medical assisting work experience

  • For Certified Pharmacy Technician, a minimum of 1 years of work experience in primary care or health plan environment
