Providence RN - Case Manager in Olympia, Washington

Providence is calling a Registered Nurse Case Manager (1.0 FTE, Days) to work in Utilization Review at Providence St. Peter Hospital in Olympia, WA.

Under supervision of Care Management Manager, works as a catalyst to promote 1) Coordination of care, 2) Continuity of Care, 3) achievement of consistent and/or expected clinical conditions, 4) patient advocacy and 5) efficient resource management. Coordinates the care for complex patients from pre-admission to immediate post-discharge. Acts in a non-caregiver capacity by facilitating coordination and communication between all members of the health care team, in the decision making process to minimize fragmentation of the health care delivery system. Assists health system assuring financial viability by performing comprehensive medical reviews utilizing appropriateness criteria and interpret implications of clinical status, medical treatment plan with current federal regulations guidelines; thereby expediting the billing process, payment and preventing governmental sanction. Effectively communicates (speaking, reading, and writing English) and resolves issues and conflicts. Utilizes strong organizational skills and understands the concept of integration and coordination of the multidisciplinary health team.

In this position you will:

  • Demonstrate expertise in the application of the nursing process. Assesses patients for care management services after a referral has been made based on the Admission screening and referrals for potential patient issues.

  • Appropriately identifiy actual and potential care issues.

  • Develop and manage a coordinated plan of care to meet patient and/or family needs.

  • Assure that patients are placed on appropriate clinical pathway and protocols, individualizes patient care needs through consultation with the multidisciplinary team.

  • Monitor the patient/family progress towards achieving optimal functional status.

  • Organize and link appropriate services within the hospital and community to deliver coordinated care to patient and family. Evaluates post-acute care environment for accessibility and adaptive needs.

  • Plan, lead, participate in or follow through with multidisciplinary team meetings as needed for care coordination. Assesses the teaching needs of the patient and family members based on patient goals and objectives.

  • Provide coordination to support the patient and family near the end of life or major life changes. Develops implements, and monitors effective discharge plan in collaboration with the patient, family, physicians, and primary caregivers. May include procuring durable medical equipment, visiting nurse services, extended care facility bed placement, support group referrals, and other services. Completes or assures a post-discharge follow-up to evaluate patient health status and level of satisfaction with care.

  • Identifiy need for new resources if gaps exist in the service continuum. Collects, analyzes, and uses care management data to continuously improve the quality of care.

  • Coordinate health education to patients, families, and staff as indicated by specific health care needs.

Qualifications:

Required qualifications for this position include:

  • 3 years of Nursing

  • Utilization Review, Home Health, Quality Management and/or Discharge Planning

  • Washington State Licensure as a Registered Nurse

Preferred qualification for this position include:

  • Bachelor's Degree in Nursing or a related field or Associate's Degree in Nursing.
