Providence Health & Services RN Care Navigator – Palliative Care Outpatient Clinic in Olympia, Washington


Providence is calling a RN Care Navigator Palliative Care Outpatient Clinic (1.0 FTE, Days) to Providence St. Peter Palliative Care Clinic in Lacey, WA. We are seeking a dedicated RN Care Navigator to support our team Monday - Thursday, 10 hour shifts.

The Palliative Care RN Care Navigator is an

integral member of the multidisciplinary Palliative Care Outpatient Team and

will work with medically complex patients and their families to provide

supportive whole person care by 1) assisting in the development of

collaborative plans of care that align with the patient’s goals 2) provide

advance care planning 3) act as a liaison between the patient’s health

care team, and 4) assist patients and their families navigate the complex

health care system. The Palliative Care RN Care Navigator facilitates

coordination and communication between the patient, members of the health care

team, and family in the decision making process to minimize fragmentation of

care throughout the patient’s journey.

In this position you will:

  • Manage a panel of patients who are actively receivingpalliative care.

  • Participate in Goals of Care conversations and provideadditional guidance and support for patients and families, as needed.

  • Work collaboratively with the palliative care providerand pharmacist to manage the patient’s medication regimen.

  • Serve as liaison between the primary provider,palliative care provider and the patient.

  • Work collaboratively with the palliative care MSW toaddress barriers for patients accessing care.

  • Work collaboratively with the interdisciplinary team toaddress the physical, emotional, social, and spiritual needs of thepatient.

  • Provide outreach and education to clinics within the 5county service area

  • Support continued growth of the outpatient palliative careservices which may include launching a new outpatient service and/oroccasionally traveling with the team to a satellite clinic.


Required qualifications for this position include:

  • Bachelor of Nursing Degree

  • Three or more years of experience as a Registered Nurse, a portion of which is in the outpatient setting

  • WA State Registered Nursing license

Preferred qualifications for this position include:

  • Hospice, oncology or hospital-based palliative care experience

  • Hospice and Palliative Care Nursing Certification or eligible and working toward certification or willing to complete certification within 2 years of hire

Washington Service Area Palliative Care Services, serves out patients from a 5

county area. The multi-disciplinary Palliative Care outpatient team is made up

of MDs, ARNPs, MA, MSW, RN Care Navigators, and a Pharmacist. The clinic is

located within the Lacey Primary & Intermediate Care Specialty Clinic in

Lacey, WA.
