Providence is calling a Pharmacist (1.0 FTE, 40 hours per week Variable Shift) to work in Pharmacy at Providence St. Peter Hospital in Olympia, WA.

The staff pharmacist has the responsibility to integrate the provision of pharmaceutical care with other elements of the patient care process to provide a collaborative, cooperative effort with other members of the health care team to assure optimal drug therapy outcomes and to improve the patient's quality of life. Performs all duties in a manner, which promotes team concept and reflects the Sisters of Providence mission and philosophy.
In this position you will:
Provides quality patient care in an efficient, caring manner. Adapts to fluctuating workload. Carries fair share of workload. Needs little or no direction in accomplishing daily work:
Completes patient care assignments within designated time parameters.
Follows shift operations check lists.
Drug orders are processed accurately in a timely manner. Utilizes information systems timely and accurately. Properly documents clinical interventions and drug information requests:
Order processing results in medication turnaround times within department standards.
Is comfortable with using Micromedex.
Completes required patient chart documentation.
Follows proper computer order entry procedures.
Mixes IV additive solutions and chemotherapy as required using aseptic technique within the laminar flow hood:
Follows all related established policies and procedures.
Reviews patient records and orders regarding drug therapy taking into consideration age and disease state. Recommends and initiates changes as appropriate; is successful in improving patient outcome through monitoring, intervention and patient education consistently. Must be competent in the assessment, treatment, and age-related care of all types of patients:
Demonstrates ability to assess patients for pharmaceutical care needs.
Can construct a pharmaceutical care plan in cooperation with other care providers
Assesses the effects of drugs prescribed and recommends changes to plan when indicated.
Checks for drug incompatibilities and allergies.
Interprets all orders to ensure drug, dosage form and dose are appropriate for the patient’s age, weight and co-morbidities.
Initiates and effectively utilizes problem solving skills to promote a positive work environment and interpersonal relationships, while maintaining professionalism and confidentiality:
Offers constructive alternatives to problems.
Listens to alternative views presented.
Attends all required staff meetings.
Maintains current knowledge of memos distributed and relevance to daily operations.
Demonstrates ongoing professionalism, including a personal commitment to advancing the profession.
Required qualifications for this position include:
Graduation from an accredited college of pharmacy
Licensed to practice as a Pharmacist within the State of Washington
Preferred qualifications for this position include:
Completion of an ASHP accredited residency program
1 year Hospital Pharmacists experience
