Providence is calling Community Health Workers (1.0 FTE, Day Shift) to improve health equity within Providence Medical Group Southwest Washington in Olympia, WA.

In this position you will:
Demonstrates Providence Core Values of respect, justice, compassion, stewardship, and excellence.
Provide outreach in the community to increase awareness of available healthcare resources.
Provides screenings of risk-factors that impact an individual’s well-being in community settings.
Document all outreach events and direct services including maintenance of log and sign-in sheets.
Required qualifications for this position include:
High School Diploma or GED.
Experience living, engaging and/or working with individual's experiencing barriers to housing, access to food, and transportation.
Valid Driver’s License and Auto Insurance.
Preferred qualifications for this position include:
Associate's or Bachelor's Degree in Healthcare or Social Work related field.
Applicants who have personal experience with barriers to healthcare.
