Providence Health & Services Care Management Social Worker - Utilization Review in Olympia, Washington

Providence is calling a Care Management and Licensed Independent Clinical Social Worker to work in Utilization Review at Providence St. Peter Hospital in Olympia, WA.

Under supervision of Care Management Manager, works as a catalyst to promote 1) Coordination of care, 2) Continuity of Care, 3) achievement of consistent and/or expected psychosocial outcomes, 4) patient advocacy and 5) efficient resource management. Coordinates the care for complex designated patients from pre-admission to immediate post-discharge. Acts in a non-caregiver capacity by facilitating coordination and communication between all member of the health care team, patient and family in the decision making process to minimize fragmentation of the health care delivery system.

In this position you will:

  • Assesses patients for care management services after a referral has been made based on the Nursing Admission screening for potential patient issues.

  • Ability to perform psychological, social, environmental, spiritual, financial, and functional assessments.

  • Ability to work independently in an unstructured environment.

  • Skill in prioritizing, time management and organization.

  • Appropriately identifies actual and potential care issues within scope of practice.

  • Critical thinking, analysis, and problem solving skills.

  • Ability to follow through on responsibilities and commitments.

  • Develops and manages a coordinated plan of care to meet care managed patient and/or family needs within scope of practice. Recognizes circumstances requiring nursing intervention.

  • Creative thinking and vision regarding possibilities for care options based on patient preferences.

  • Identifies and acts upon barriers to wellness.

  • Determines patient resources and determines availability of financial means/insurance benefits.

  • Strong verbal and written communication skills.

  • Utilizes knowledge of protocols and guidelines.

  • Monitors the patient/family progress toward goal achievement of optimizing functional status.

  • Strong collaboration and team skills, including the ability to work well with physicians, all disciplines and staff levels.

Qualifications:

Required qualifications for this position include:

  • Master's Degree in Social Work or related field.

  • Three (3) years social services experience with a health care related organization, preferably in acute care/ care coordination
