Providence Registered Nurse (RN) Specialist in Monroe, Washington
Providence is calling a Registered Nurse Care Specialist (1.0 FTE, Day Shift) to work in Pmg Managed Care Support at Providence Medical Group Northwest Washington in Monroe, WA.
The RN Specialist is responsible for managing patient populations with chronic illness and/or multiple co-morbidities. This individual works in coordination with multiple PCPs and serves as the central link to an identified population of patients. Through skilled assessment, education, collaboration and coordination of healthcare and community resources, the RN Specialist assists patients to gain self efficacy/management skills, achieve optimum functional health status, and quality of life. The RN Specialist assists patients/families, staff and systems to achieve high quality, evidence based, cost-effective, and patient focused outcomes. This person works under minimal supervision within the general policies and standard practices and the nursing code of ethics , including Chapter 18.88 RCW. May perform other ambulatory care RN duties as required.
In this position you will:
Provide leadership and continuity of care guidance for patients with chronic diseases and/or complex disease states. Uses knowledge of illness, diagnoses and treatments in comprehensive, holistic assessments of patients. Performs patient teaching, patient self-management/self-efficacy coaching and planned care management. Ensures coordination for specialty and other handoffs as necessary.
Utilizes appropriate resources to gather data and assess patient care needs; makes ongoing assessments of the following domains: physio-biological, cognitive/emotional, spiritual and social, managing life with chronic disease; identifies problems that could lead to an emergency/crisis situation and takes appropriate action to de-escalate; identifies the need for and coordinates specialists/consultants; communicates assessment data to appropriate persons; assesses learning needs of patient/significant others; documents assessment data according to clinic standards;
Plans for and manages a panel of patients with chronic/complex disease(s); plans interventions appropriate to patients’ medical and nursing diagnoses, age, abilities and resources; establishes and implements learning and disease self-management plan for patient/significant others; documents planning process according to organizational standards;
Provides teaching to patients/families related to patient’s diagnosis, pathology, medical and nursing treatment plans, discharge needs and health goals; documents each element of care per organizational and unit standards.
Collaboration, delegation, supervision:
Collaborates and communicates care with patient’s PCP;
Forms partnerships with all members of the interdisciplinary team;
Communicates with coworkers and other departments in a professional manner; shares appropriate information with all members of the health care team, including assistive/support personnel, in a manner that is appropriate and timely for the patient’s condition;
Participates in team huddles;
Monitors patient and family satisfaction.

Supports and guides ambulatory health care teams with implementation of evidence –based practice guidelines.
Assists in designing and implementing planned care interventions through a combination of evidence-based guideline/pathway utilization and individualized care planning.
Other ambulatory care RN duties as assigned.
Required qualifications for this position include:
2 years of work experience in an RN role
Currently licensed as a Registered Nurse (RN) in the State of Washington.
Resuscitation Quality Improvement (RQI)
Preferred qualifications for this position include:
Experience managing complex patients in an ambulatory setting
Medical Home Model experience
Certification of diabetes education (CDE)
