Providence is calling an RN Wound Care Specialist ( Home Health, Day Shift, 1.0 FTE) to Providence SoundHomeCare and Hospice in Lacey, Washington.

In this position you will:
Serve as a clinical resource to agency staff on issues pertaining to patients with wounds, pressure ulcers, ostomies (if area of expertise), fistulas and incontinence (if area of expertise). Makes joint visits for evaluation and education purposes as needed.
Perform comprehensive assessments, develop plans of care, coordinate care with other disciplines and provide direct patient care in the implementation of outcome oriented interventions under the physician ordered Plan of Care.
Perform OASIS comprehensive assessment primarily for patients with wounds and ostomies.
Promote and deliver coordinated wound, ostomy, and continence services per certification held.
Promote comprehensive care for patients through coordination of medical, agency, and community resources. Assists patients with preparation for discharge including supplies needed.
Collaborate with all healthcare team members (internal & external) toward the best outcomes for the patient and ministry.
Assess POC of ostomy patients (if area of expertise) and promotes self-care and psychosocial rehabilitation. Evaluates the effectiveness of care provided and revises care plan if indicated.
Serve as an educator to patients and their families, staff, physicians, and other healthcare professionals in the community regarding the management of ostomies, fistulas, wound, pressure ulcers, and incontinence (up to level of certification). This may include in-service education and orientation classes.
Required qualifications for this position include:
Graduate of an accredited school of nursing
Graduate of a certification program provided by Wound Ostomy Continence Nursing Certification Board (WOCNCB), National Alliance of Wound Care and Ostomy (NAWCO), American Board of Wound Management (ABWM)
Current Registered Nurse Washington State License
Current certification in one of the following: CWOCN (Certified Wound Ostomy Continence Nurse), WOCN (Certified Wound Ostomy Nurse), CWCN (Certified Wound Care Nurse) , WCC (Wound Care Certified) , CWS (Certified Wound Specialist), CWCA (Certified Wound Care Associate)
Current Provider Basic Life Support (BLS) certification
Current Driver’s license and have access to daily use of an insured, reliable vehicle
2 years of RN experience
Experience managing simple and complex wounds
Preferred qualifications for this position include:
2 years of direct-patient experience with wound certification
Experience in a Medicare-certified Homecare or Hospice agency/facility
Experience managing and teaching ostomy care
