Providence Health & Services Physical Therapy Assistant - Home Health Sound (0.0 FTE, Days) in Lacey, Washington


Providence is calling a Physical Therapy Assistant (Home Health Sound, Day Shift, 0.0 FTE) to SCS Home Health in Lacey, Washington. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all - especially those who are poor and vulnerable.

In this position you will have the following responsibilities:

  • PT care needs are assessed as needed for PT patients

  • After a referral, an accurate history and physical exam are performed

  • Document assessment data within 24 hours of home visit

  • Describe assessment and accurately interprets data holistically (psychological, physiological, cognitive and medication regime)

  • Observe reimbursement eligibility criteria in implementing services for patients

  • Assist in obtaining information to determine alternative payment sources

  • Communicate with authorization case management to determine eligibility for home care

  • Determine priorities of client care based on client's needs and available resources

  • Manage time effectively.

  • Prioritize client's needs effectively

  • Written plan of care developed which best meets the client's needs holistically. Defines the coordination of care needed

  • Cooperatively assists patients and significant others in developing a realistic plan of care

  • Develop a patient plan of care and supervises client's care on an individual basis as applies to Assistants/Aides regarding PT


Required qualifications for this position includes:

  • Associate's Degree in Physical Therapy Assistance -or- a graduate of an approved school of physical therapy assistant who is eligible for licensure

  • Current Washington State Physical Therapy Assistant License

  • Valid Washington State driver’s license

  • Insured transportation

  • Healthcare Provider BLS/CPR certification

  • One (1) year of experience in an acute care facility or a graduate of an approved school of physical therapy assistant who is eligible for licensure.
