Providence Occupational Therapist - Home Health in Lacey, Washington
Providence is calling an Occupational Therapist (OT) (Home Health, Day Shift, 1.0 FTE) to Providence SoundHomeCare and Hospice in Lacey, Washington. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all - especially those who are poor and vulnerable.
In this position you will:
Describes assessment & accurately interprets data holistically (psychological, physiological, cognitive & medication regime).
Observes reimbursement eligibility criteria in implementing services for clients
Assists in obtaining information to determine alternative payment sources.
Communicates with insurance co. or HMO's to determine eligibility for home care.
Written plan of care developed which best meets the client's needs holistically. Defines the coordination of care needed.
Cooperatively assists clients & significant others in developing a realistic plan of care.
Develops a patient plan of care & supervises client's care on an individual basis as applies to Assistants/Aides regarding OT
Completes Agency documentation such as daily schedules, time sheets, mileage reports per SHC guidelines, unusual occurrence, employee injury and paid time off requests
Required qualifications for this position include:
Bachelor’s degree in Occupational Therapy from an institution approved by the state board in collaboration with the American Occupational Therapy Association
Current Washington State Occupational Therapist License
Current Healthcare Provider Cardiopulmonary Resuscitation (CPR) certification
Current Washington State driver’s license and access to daily use of an insured, reliable vehicle or an alternate ability to travel to sites as required
Proof of current auto insurance
Preferred qualifications for this position include:
- Experience in an acute care facility preferred.
