Providence Health & Services Ambulatory Oncology Clinical Pharmacist – PRCS in Lacey, Washington
Ambulatory Oncology Clinical Pharmacist – Providence Regional Cancer Systems: Providence is calling an ambulatory oncology clinical pharmacist (1.0FTE) to Providence Southwest Region in Washington State.
The Outpatient Infusion Clinical Pharmacist will provide routine clinical activities and supervise compounding of sterile preparations within the PRCS clinic pharmacies. The Pharmacist will provide consultations to providers, review treatment plans and perform education to patients/family and other healthcare professionals. While participating in developing and monitoring patients chemotherapy regimens and other supportive therapy, the pharmacist will maintain the mission and core values of the Sisters of Providence. This is a full time position with varying hours based predominantly in the Lacey clinic, but with varying locations. The clinics are administered under Providence Centralia Hospital and are located in Lacey, Centralia and Aberdeen.
In this position you will:
Conduct thorough patient profile reviews to monitor for adverse events and assess the appropriateness of prescribed therapies.
Evaluate all provider orders and verify correctness of entry in the pharmacy computer system and computerized patient profiles.
Participate in collaborative drug therapy management.
Ensure knowledge of physiology and pathophysiology of managed disease conditions.
Participate in teaching of APPE students and pharmacy residents.
Ensure correct preparation, labeling, and delivery of medications including compounded sterile products.
Direct pharmacy technicians in all areas of technical responsibilities including supervision of proper garbing, handwashing, cleaning, and aseptic processes.
Provide education to patients, physicians, nurses, and other members of the healthcare team.
Participate in protocol development, exploratory projects and committees which contribute to improved patient care.
Document clinical interventions as appropriate.
Required qualifications for this position include:
Doctorate of Pharmacy degree from an accredited school of Pharmacy
Current registration and license from Washington State Board of Pharmacy
BLS certification must be obtained within 6 months of hire
Completed ASHP-accredited PGY-1 Residency program or 3 years of work experience
BCOP certification must be obtained within 3 years of hire
Preferred qualifications for this position include:
- Completed ASHP-accredited PGY-2 Residency program in Oncology or equivalent work experience
