Providence is calling a Social Worker MSW (1.0 FTE, Day Shift) to work in the Hospice Snohomish at Senior & Community Services Hospice in Everett, WA
This position is responsible for providing services to hospice clients, families, and caregivers, addressing a wide range of psychosocial issues related to end-of-life needs including: adjustment to illness/prognosis, care system issues, community resource issues, insurance/financial issues, legal/ethical issues, and end-of-life planning. The MSW develops and implements a plan of care in collaboration with the Hospice Interdisciplinary Team.
In this position you will:
Provides medical social work service to patients and families including assessment, counseling, community resource referral, advocacy, crisis intervention, and discharge planning utilizing current practice knowledge and skill.
Consults and educates staff, volunteers, and community of the social and emotional factors related to the End of Life Care.
Provides social work in accordance with the NASW Code of Ethics and Providence Hospice & Home Care of Snohomish County’s philosophy, policies and procedures.
All required documentation will be complete, accurate and timely.
Promotes and practices patient/family, personal and environmental safety while performing job duties according to agency policies and procedures.
Participates in relevant educational activities to provide competent care to patients/caregivers, to improve the social work skills, and to seek personal and professional growth.
Displays professionalism in the work environment in all areas including communication with both internal and external customers.
Is knowledgeable with Hospice philosophy, social work assessments and interventions (spiritual) related to coping, palliative care, grief and quality of life issues for patient/families receiving Hospice care.
Complies with all applicable Federal, State, and local laws, regulations, and CHAP requirements as well as facility specific policies and procedures.
Maintain confidentiality of all patient demographics, medical and financial information at all times. Maintain confidentiality of all company and client information. Ensure proper handling and disposal of confidential documents and adheres to all HIPAA rules and regulations.
Adheres to all policies and procedures including regulatory requirements with regard to infection control, practicing universal precautions, sanitation, and safety and disaster preparedness.
Required qualifications for this position includes:
Master's Degree In Social Work from an accredited School of Social Work.
1 year Health care setting.
Current WA Registered Counselor/Social Worker Licensure.
Current Healthcare Provider CPR certification.
Valid Washington State driver’s license and have access to daily use of an insured reliable vehicle or an alternate ability to travel to sites as required.
Proof of current Auto Insurance.
