Providence is calling a Registered Nurse (RN) Triage (Per Diem, Weekend Days) to Home Health Snohomish in Everett, Washington.

In this position you will:
Assess the needs of patients and families by phone, with special attention to the symptoms and comfort of the patient; provide immediate problem-solving support.
Contact covering MD’s to obtain needed medications, arrange for nursing visit if needed, and/or provide follow-up communication to other staff regarding patient/family needs.
Implement nursing interventions based on the Plan of Care and Home Health policies and procedures.
Anticipate disease progression and implications for symptom management.
Inform patient and family of current and anticipated changes in patient condition as appropriate.
Instruct patient and family in all aspects of care.
Required qualifications for this position include:
Associate's or Bachelor's degree in Nursing
Current Washington State RN license
3 years of Nursing experience
Current Washington State Driver's license
Current Healthcare Provider CPR Certification
Preferred qualifications for this position include:
Experience in Home Health, Hospice, Long Term Care, and/or acute care
Ability to work Saturday and Sundays
