Providence Health & Services Pharmacist - Anticoagulation in Everett, Washington
Providence is calling a Pharmacist (0.5FTE, variable shifts) to our Anticoagulation Clinics in Everett, Mill Creek, and Monroe Washington.
We are seeking a Pharmacist, Anticoag to perform professional pharmaceutical dispensing functions by providing prescribed medications, drugs, and other pharmaceuticals according to standards and practices for patients in all age groups including neonatal, pediatric, adolescent, adult and geriatric. He or she will be expected to promote safe, cost-effective, rational drug use and provide expert pharmaceutical consultation to other healthcare colleagues.
In this position you will:
Communicate effectively both orally and in writing.
Be able to establish and maintain priorities for responding to requests and follow established policies.
Maintain confidentiality of records or medical center information at all times; maintain patient and employer confidentiality.
Perform research and analysis to provide financial data and recommendations for use in setting and realizing financial objectives.
Ensure compliance with JCAHO, State and Federal regulatory agencies and PEMC standards and policies.
Required qualifications for this position include:
Graduate of an accredited school of Pharmacy.
Licensed as a Registered Pharmacist in the State of Washington.
BLS required within 2 weeks of first day of work.
Knowledge of community clinical pharmacy practice including drug therapy standards, therapeutic outcomes monitoring, drug use control, drug utilization review, formulary development and management, adverse drug reaction management, pharmacokinetics, and nutritional support.
Knowledge of technical and quality assurance processes utilized in pharmacy practice, ambulatory clinical pharmacy practice, patient care systems, and therapeutic collaboration with prescriptive; knowledge with formulary and disease state management.
