Ambulatory Oncology Clinical Pharmacist – ProvidenceRegional Cancer Systems: Providence is calling an ambulatory oncology clinical pharmacist (0.6FTE) to
Providence Southwest Region in Washington State. This is a part-time position with varying hours based in our
Centralia and Aberdeen clinic locations. The clinics are administered under
Providence Centralia Hospital and are located in Lacey, Centralia and
The Outpatient Infusion Clinical Pharmacist will provide routine clinical
activities and supervise compounding of sterile preparations within the PRCS
clinic pharmacies. The Pharmacist will provide consultations to providers,
review treatment plans and perform education to patients/family and other
healthcare professionals. While
participating in developing and monitoring patients chemotherapy regimens and
other supportive therapy, the pharmacist will maintain the mission and core
values of the Sisters of Providence. This is a full time position with varying
hours based predominantly in the Lacey clinic, but with varying locations. The
clinics are administered under Providence Centralia Hospital and are located in
Lacey, Centralia and Aberdeen.
In this position you will:
Conduct thorough patientprofile reviews to monitor for adverse events and assess theappropriateness of prescribed therapies.
Evaluate all providerorders and verify correctness of entry in the pharmacy computer system andcomputerized patient profiles.
Participate incollaborative drug therapy management.
Ensure knowledge ofphysiology and pathophysiology of managed disease conditions.
Participate in teaching ofAPPE students and pharmacy residents.
Ensure correctpreparation, labeling, and delivery of medications including compoundedsterile products.
Direct pharmacytechnicians in all areas of technical responsibilities includingsupervision of proper garbing, handwashing, cleaning, and asepticprocesses.
Provide education topatients, physicians, nurses, and other members of the healthcare team.
Participate in protocoldevelopment, exploratory projects and committees which contribute toimproved patient care.
Document clinicalinterventions as appropriate.
Required qualifications for this position include:
Doctorate of Pharmacy degree from an accredited school ofPharmacy
Current registration and license from Washington State Boardof Pharmacy
BLS certification must be obtained within 6 months of hire
Completed ASHP-accredited PGY-1 Residency program or 3 yearsof work experience
BCOP certification must be obtained within 3 years of hire
Preferred qualifications for this position include:
- Completed ASHP-accredited PGY-2 Residency program inOncology or equivalent work experience
